Значение N-концевого пропептида проколлагена III типа для прогнозирования диастолической дисфункции у пациентов с инфарктом миокарда и сохраненной фракцией выброса

Определить особенности динамики биохимического марке- ра фиброзирования N-концевого пропептида проколлагена III типа (PIIINP) и структурно-функциональных показателей с оценкой диа- столической функции у пациентов через год после инфаркта мио- карда (ИМ) с подъемом сегмента ST (ИМпST) и сохраненной сокра- тительной способностью левого желудочка (ЛЖ). Материал и методы. В исследование сплошным методом включе- ны 120 (100%) больных ИМпST. Далее отобраны пациенты с фракцией выброса (ФВ) ЛЖ ≥50%. В окончательный анализ включены 86 па- циентов с ИМпST. При поступлении в стационар больным проводи- лись стандартные лабораторные и инструментальные обследования, коронарография со стентированием симптом-зависимой артерии. Эхокардиографическое исследование, определение концентрации PIIINP и N-концевого фрагмента мозгового натрийуретического про- пептида (NT-proBNP) в сыворотке венозной крови проводилось на первые (точка 1) и 12-е сут. (точка 2) заболевания и через год (точка 3). Для сравнения полученных значений маркеров фиброзирования сформирована группа контроля, 20 (100%) здоровых добровольцев, тождественная по возрасту и полу с изучаемой выборкой. Результаты. В первые сутки ИМ выявлено 25 (29,1%) пациентов с признаками диастолической дисфункции (ДД) среди лиц с сохра- ненной ФВ ЛЖ. Через 1 год отмечен рост числа таких пациентов на 10% (n=9).
